On March 31, 1993, Lee was filming a scene for the film The Crow in which his character is shot by thugs. In the scene, Lee’s character walks into her apartment and discovers that her fiancé is being beaten and raped, and Smith is shot by a thug played by actor Michael Massey.
In the film shooting before the fatal scene, the gun that had been used as a prop (an actual revolver) was loaded with improperly made dummy rounds, improvised from live cartridges, including special The powder charge was removed by the impact crew, so a close-up revolver would show normal-looking ammunition. However, the crew neglected to remove the primers from the cartridge, and at some point before the fatal incident, one round was fired. Although there was no powder charge, the energy from the ignited primer was sufficient to detach the bullet from the casing and push it partially into the gun barrel, where it was trapped – a dangerous condition known as a squib load.
